The Centre of Excellence for Information Sharing recently delivered a workshop to key partners from Surrey’s Mental Health Crisis Care Concordat Delivery Group (CCDG).  Learn more about the Crisis Care Concordat

We are worked closely with key partners across Surrey to support their work and included support on information sharing to identify cohorts at risk, share crisis contingency plans, identify frequent callers and track outcomes of interventions.  The next phase developed a full understanding of the information sharing challenges for the project through exploring individuals experience of crisis response, developed case studies, established the information needs of a Single Point of Access and produced a proposal to support organisational and individual culture change around information sharing.

The Centre gave a presentation at the Public Sector Show 2015 on how information sharing is critical to successful service transformation, which you can watch below:

The next step was to delivered a workshop to key partners from Surrey’s Mental Health Crisis Care Concordat Delivery Group (CCDG). We supported a broad range of partners and service users represented at the Surrey CCDG as they work collaboratively to deliver outcomes for Surrey’s Crisis Care Concordat and transform services to realise better outcomes for those experiencing mental health crises.

The workshop was an opportunity for the CCDG to find out about how the Centre supports local areas to identify cultural barriers and solutions that are fundamental to developing effective information sharing.
